If you require prosthetic care, please complete the application.The form must be completed personally by the patient. For children under 18, the form must be completed by a legal guardian or an authorized representative.
Please note that Future for Ukraine does not cover any participant expenses, including logistics (travel costs), accommodation, meals, or other related costs during participation in the project.
